Care Coordinator II (LPN Required)

JOB TITLE:   Care Coordinator II (LPN Required)

DEPARTMENT: Integrated Behavioral Health Services

FLSA:  Full-Time, Exempt

DURATION:  Open Until Filled

SALARY RANGE:  $40,000-$50,000/annually

 

POSITION PURPOSE:

Implementing and building personal care plans and coordinating chronic care management needs.

Assist/advise families with available service assessments.

Tracks in-patients at hospitals.

Input accurate and complete data for all contacts with clients into agency EMR and other databases.

Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, client contact sheets, and evaluations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Implementing and building personal care plans and coordinating chronic care management needs

  • Assist/advise families with available service assessments.

  • Tracks in-patients at hospitals.

  • Input accurate and complete data for all contacts with clients into agency EMR and other databases

  • Schedule and confirm appointments for patients at specialty clinics, as necessary.

  • Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, client contact sheets, and evaluations.

  • Reminding patients of appointments.

  • Maintain the patient’s PCP (personal care plan) and keeps appropriate members of the health care team informed about relevant issues concerning the patient’s care and discharge plans

  • In consultation with the patient’s PCP, identify and reconcile discrepancies, contradictions, and oversights among medical orders—paying special attention to medication orders

  • Actively participates in continuing quality improvement activities.

  • Performs telephone outreach and follows the scripted process outlined for providing or educating patients, their family and caretakers regarding chronic care self-management, independent living, and activities of daily living per personal care plans.

  • Supervising the Care Coordinator I and Certified Medical Assistants

  • Other duties as assigned

EDUCATION AND QUALIFICATIONS:

Licensed Practical Nurse (LPN) required. Minimum of 3 years working in a primary care medical practice strongly preferred.  

REQUIRED KNOWLEDGE:  

Ability to manage clinic flow of patients and apply time management principles.

SKILLS/ABILITIES:  

Must be able to maintain confidentiality; Strong computer skills; Ability to multi-task and be organized; Strong oral, written, and interpersonal communications skills; bi-lingual (Spanish-speaking) desirable.
