CLINICAL MEDICAL ASSISTANT (Certification Required)

Performs Clinical duties as they pertain outpatient clinical care. Assists provider (Physician/Nurse Practitioner/Physician Assistant) with medical procedures and tests in conjunction with patient visits. Position will participate in QI activities as needed.

CASE MANAGER - SOAR (Adults)

The Case Manager is responsible for developing and monitoring the case plans of adult program participants in the SOAR (Skills, Opportunity, Achievement, Responsibility) Re-entry Program. The incumbent duties require close coordination, collaboration, tracking and monitoring of participants to ensure compliance with their service plans. Responsibilities also include working closely with partner agencies to ensure program participants are engaged and supported in services that include employment/training, educational attainment, and other supportive services.

PHARMACY TECHNICIAN

The Pharmacy Technician will be responsible for the management of the pharmacy patient service area and all related activities. The Pharmacy Technician will also assist the Pharmacist during the dispensing process of prescriptions upon direction of the Pharmacist. Position will participate in QI activities as needed.

PATIENT SERVICES REPRESENTATIVE (Bilingual Preferred)

The Patient Services Representative is responsible for inputting complete, correct patient demographic information, updating all patient financial data, verifying and documenting patient benefits on each account. Position will participate in QI activities as needed.

HR PAYROLL SPECIALIST

The HR Payroll Specialist will perform tasks related primary to payroll processing. These tasks will include but are not limited to data entry into the Human Resources Information System (HRIS), verification of timecard/timesheet data, leave request tracking and clerical/filing. This position will perform the first check for accuracy for all payroll submissions.

NURSE PRACTITIONER AND/OR PHYSICIAN ASSISTANT

Responsible for health care coordination for patients in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Position will participate in QI activities as needed.

STAFF PHYSICIAN

The Staff Physician provides direct care services to patients within an urgent care environment. Injuries addressed will be those requiring immediate medical attention, but not serious enough to require an Emergency Department visit (such as various sports related injuries, etc.).

CARE COORDINATOR II (LPN Required)

Implementing and building personal care plans and coordinating chronic care management needs.

Assist/advise families with available service assessments.

Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, client contact sheets, and evaluations.

BEHAVIORAL HEALTH CLINICIAN (LCSW Required)

Responsible for assessment and treatment of individuals with mental, emotional, social or substance abuse problems, including abuse of alcohol, tobacco, and/or other related drugs. Activities may include individual, group therapy, crisis intervention, case management, client advocacy, prevention, and education. Ensures compliance with current federal and state standards, guidelines and regulations.  Position will participate in QI activities as needed.

INFORMATION TECHNOLOGY MANAGER

Plans, directs and manages the Information Technology activities for the organization. Areas of technical responsibility may include: computer hardware and software applications, networks databases, data processing, computer programming, system security, project management, user system training and help desk. Responsible for managing budgets, planning and systems maintenance. Position will supervise other IT staff.
