NURSE PRACTITIONER AND/OR PHYSICIAN ASSISTANT

Responsible for health care coordination for patients in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Position will participate in QI activities as needed.

STAFF PHYSICIAN

The Staff Physician provides direct care services to patients within an urgent care environment. Injuries addressed will be those requiring immediate medical attention, but not serious enough to require an Emergency Department visit (such as various sports related injuries, etc.).

REFERRAL SPECIALIST

The Referral Specialist will assist patients not able to access such specialty care as matter of insurance or other barrier. The Referral Specialist will coordinate the referral made by the Provider to the most effective specialty or required care and provide ongoing follow up on patients’ status inside and outside the health care arena.  The Referral Specialist will participate in QI activities as needed.

DENTAL ASSISTANT II

Supports dental care delivery by preparing treatment room, patient, instruments, and materials; passing instruments and materials; performing procedures in compliance with the dental practice act.  Position will participate in QI activities as needed.

CARE COORDINATOR II (LPN Required)

Implementing and building personal care plans and coordinating chronic care management needs.

Assist/advise families with available service assessments.

Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, client contact sheets, and evaluations.

BEHAVIORAL HEALTH CLINICIAN (LCSW Required)

Responsible for assessment and treatment of individuals with mental, emotional, social or substance abuse problems, including abuse of alcohol, tobacco, and/or other related drugs. Activities may include individual, group therapy, crisis intervention, case management, client advocacy, prevention, and education. Ensures compliance with current federal and state standards, guidelines and regulations.  Position will participate in QI activities as needed.

COMMUNITY OUTREACH/HEALTH WORKER (Bridge Counselor)

The Community Outreach/Health Worker (Bridge Counselor) will provide screening, patient coordination, follow-up, education, and other outreach services specifically for patients diagnosed with HIV, Hepatitis C, and Hepatitis B.  Position will provide medical, ancillary case management services, as well as linkage to care and community resources.

INFORMATION TECHNOLOGY MANAGER

Plans, directs and manages the Information Technology activities for the organization. Areas of technical responsibility may include: computer hardware and software applications, networks databases, data processing, computer programming, system security, project management, user system training and help desk. Responsible for managing budgets, planning and systems maintenance. Position will supervise other IT staff.

QUALITY IMPROVEMENT COORDINATOR (RN OR LPN REQUIRED)

The Quality Coordinator supports the activities of the Quality Improvement (QI) Program for the clinic. The primary responsibility of the quality improvement coordinator is to ensure that all staff complies with quality, service performance and medical record policies and procedures.
